The Consumer Directed Personal Assistance Program (CDPAP) offers individuals who meet the CDPAP eligibility requirements greater freedom in choosing their quality of care.

New York State CDPAP eligibility’s primary requirement is that Medicaid consumers can make independent decisions about their care. They may also have a legal guardian, designated relative, or another adult willing and capable of making these decisions.

Eligibility to receive medical assistance and long-term care services, being in a stable medical condition, needing partial or complete personal care, home health, or skilled nursing tasks, and being able to participate in assessments and reassessments.

If you meet these requirements, your doctor must send a completed Physician’s Order for Services to your local social services district, and the social services district will conduct a social and nursing assessment, including a personal nursing assessment, to determine if you can participate in CDPAP and will recommend the amount, frequency, and duration of services.

Once approved for the CDPAP program, you can choose an agency or vendor to provide CDPAP services. Our home care agency is a licensed CDPAP services provider in New York State, allowing you to select your own caregivers and direct the care.

CDPAP caregivers must meet eligibility requirements, including being at least 18 years old and authorized to work in the United States. Additionally, they must agree to undergo an initial health assessment by the New York Department of Health.

Close relatives such as spouses or custodial parents cannot serve as paid CDPAP caregivers to avoid potential conflicts of interest.
