After you have submitted the CDPAP form or provided us with the necessary information, we will review the patient’s eligibility and assign one of our intake specialists to the case. They will contact you to discuss the next steps and answer any questions you may have.
The assigned coordinator will assist the patient in completing a physician’s order for homecare services. Following this, the patient will need to undergo a social and nursing assessment conducted by the state to determine health status and eligibility for CDPAP and home care services.
Based on these assessments, the Medicaid Long Term Care plan will recommend the appropriate amount, frequency, and duration of services for the Medicaid recipient.
CDPAP caregiver requirements:
In order to participate in the CDPAP program, CDPAP caregivers must meet certain requirements. They must be at least 18 years of age and authorized to work in the United States. Additionally, they must consent to an initial mandatory health assessment, as per the New York Department of Health.
The CDPAP program enables individuals who meet its eligibility criteria to have more autonomy in selecting the quality of care they receive.
