Managed Long Term Care Plan (MLTC) provides quality health care services to Medicaid-eligible adults in need of Community Based Long Term Services and Support. MLTC offers access to benefits and services that help members live healthier, more productive lives. A team of healthcare professionals works collaboratively with members, their families/representatives, and their providers to create a plan of Care tailored to the specific needs of each member. Every member is Assigned a care management team whose responsibilities include coordinating healthcare services, conducting home visits to monitor health status, and assisting with social and environmental services.
WHO IS ELIGIBLE FOR MANAGED LONG TERM CARE PLAN (MLTCP)?
Eligibility depends on several criteria, which include but are not limited to the following:
- Must be at least 21 years old.
- Must live in New York State
- Must need more than 120 days of Community Based Long term Care Services.
- Must be Medicaid eligible
